NHS Sefton Community Health Services are redesigning service provision for our patients who have a long term condition.  This service redesign will be facilitated by NHS Northwest Improvement Foundation in partnership with ourselves and  our Commissioners. 

You are invited to attend the launch event for this programme (please see attached agenda). This event is to be held on: Thursday 11th February 2010  At: Waterloo Rugby Club, St Anthonys Road, Blundellsands, L23 8TW  The event will start at 1.30pm and continue until 4.30pm however, registration and refreshments are available from1pm.
